Misconceptions about the diseases, its treatment and Prevention. prevailing among people are the important cause for the claim of miracle cure in certain diseases . This is most common in underdeveloped countries, some what common in developing countries and uncommon in developed countries.

There are different system of therapies available to treat the diseases. This may be based scientifically or in other ways.

In each system of therapy, practitioners can claim the cure for a particular  diseased conditions with a particular type of therapy. They will claim,the cure based on  reducing the signs and symptoms  as miracle  without treating the real causes.

Most of the functional psychiatric disorders are claimed as cured by magics and miracles.

Certain  incurable conditions are claimed as cured with plant and animal products.

3. கபாடபுரத்திற்கு நேர்ந்த கடல்கோளின் பின்னர் இன்றைய மதுரை நகரத்திற்குப் பாண்டியர்கள் இடம்பெயர்ந்தனர். அங்கே தோற்றுவித்து வளர்க்கப்பட்டதே கடைச்சங்கம். கடைச்சங்கத்தின் காலம் கிமு இரண்டாம் நூற்றாண்டு என்பதற்கே பலர் பல்வேறு குறுக்கு வழக்குகளோடு வருவர். சான்றெங்கே, ஆதாரம் எங்கே என்று நிற்பர். இப்போது கிமு ஆறாம் நூற்றாண்டுத் தமிழ் எழுத்துகள் தெளிந்த சான்றுகளாகிவிட்டன.

4. ஆதன், சாத்தன் ஆகிய பெயர்கள் நம் இலக்கண உரைகளில் தொடர்ந்து மேற்கோள் காட்டப்படுகின்றன. ஆதனின் தந்தை ஆந்தை எனப்படுவார். சாத்தனின் தந்தை சாத்தந்தை எனப்படுவார். பிசிர் என்ற ஊரில் வாழ்ந்த ஆதனின் தந்தையே பிசிர் ஆந்தையார் எனப்பட்டார். அகநானூறு, புறநானூறு, நற்றிணை ஆகிய சங்க இலக்கிய நூல்களில் பிசிராந்தையார் பாடிய ஆறு பாடல்கள் இடம்பெற்றிருக்கின்றன. அவர் யாராயினும் ஆதன் என்ற பெயர் வைக்கும் பழக்கம் தொல் தமிழரிடையே பரந்திருந்தது என்பது வெள்ளிடைமலை. ஆதன் என்பதற்கு உயிர் என்று பொருள். உயிரன்.

5. ஒடிய மாநிலம் புவனேசுவரம் உதயகிரிக் குகைகளின் நெற்றியில் பொறிக்கப்பட்டுள்ள காரவேலனின் கல்வெட்டு பதின்மூன்று நூற்றாண்டுகளாய் நிலவிய சேர சோழ பாண்டியர்களின் கூட்டாட்சி வலிமையைக் கூறுகிறது. “தமிர தேக சங்காத்தம்” என்பது அக்கல்வெட்டினில் உள்ள தொடர். ஒடிய மன்னன் காரவேலன் அசோகருக்குப் பிறகு அப்பகுதியினை ஆண்டவன். கிமு இரண்டாம் நூற்றாண்டினைச் சேர்ந்தவன். “அக்கல்வெட்டினில் இருப்பது பதின்மூன்று நூற்றாண்டுகள் இல்லை, வெறும் பதின்மூன்று ஆண்டுகளாகத்தான் இருக்கவேண்டும்” என்ற வழக்கும் ஓடியது. கீழடியில் பெருந்தமிழர் நாகரிகம் செழித்து வளர்ந்திருக்கும் நிலைமையைக் காண்கையில் காரவேலன் கல்வெட்டு கூறுவது பதின்மூன்று நூற்றாண்டுகளாகவே இருக்க வேண்டும்.

6. வைகை ஆறு இரண்டாயிரம் ஆண்டுகட்கு முன்பு பேராறாக நிறைந்து ஓடியிருக்க வேண்டும். அதன் கரைவெளி எங்கும் பாண்டியப் பேரரசில் பெருவாழ்வு வாழ்ந்த குடிகளின் தடயங்களைக் கண்டெடுத்திருக்கிறோம்.

7. எழுத்துமுறை தோன்றுவதற்கு முன்பே பல்லாயிரம் ஆண்டுகளாகப் பேச்சுமுறை நிலவியிருக்க வேண்டும். மொழித்தோற்றத்தின் இளமைக் காலம் இன்னும் பல நூற்றாண்டுகள் பின்னோக்கிச் செல்லும்.
அனைத்தையும் கொண்டு கூட்டிப் பார்க்கையில் தமிழ் மொழியின் தொன்மையைப் பகரும் சான்றுகள் பல பல்லாயிரம் ஆண்டுகட்கு முந்தியவை என்பது தெளிவாக நம் கண்ணுக்கே தெரிகிறது.

8. கீழடியில் தங்க அணிகள் கிடைத்திருக்கின்றன. தொலைவுத் தேயங்களிலிருந்து வருவிக்கப்பட்ட மணிகள் கிடைத்திருக்கின்றன. மண்ணைக் கொண்டு பாண்டங்கள் செய்தல் என்னும் தொழில்நுட்பம் சிறப்படைந்திருக்கிறது. இருப்புப் பொருள்களும் பல்வேறு மாழைப் பொருள்களும் (உலோகம்) பயன்பாட்டுக்கு வந்திருக்கின்றன. தனிப்பான்மையான குடிவாழ்வின் தன்னிகரற்ற வரலாற்று வளர்ச்சி நிலைகள் இவை.

9 இன்றைய நிலைப்பாட்டிலிருந்து வரலாற்றினைப் பார்ப்பது தவறு. மதங்கள், சாதிகள், பழக்கவழக்கங்கள், பண்பாடுகள் என இன்று நாம் பற்றிப் பழகியிருக்கும் சிறுகண்களைக்கொண்டு பழைமையில் தேடுவது நன்றன்று. ஒற்றை நிலையில் ஒரு நிலத்தின் வரலாறும் அமையாது. காலப்போக்கில் அது பல்வேறு நிலைகளுக்கு முகங்கொடுத்தாக வேண்டும். நம் வரலாறெங்கும் அவ்வாறே நிகழ்ந்தது. எது எப்படியாயினும் அன்றைக்கும் இன்றைக்கும் இக்குடிவாழ்வின் பற்றுதலாக இருப்பது ஒன்றேயொன்று. அதுதான் முன்னைப் பழையதும் பின்னைப் புதியதுமான தமிழ்மொழி !

Though the exact number of snake bites is unknown, an estimated 5.4 million people are bitten each year with up to 2.7 million envenomings.


Around 81 000 to 138 000 people die each year because of snake bites, and around three times as many amputations and other permanent disabilities are caused by snakebites annually.


Bites by venomous snakes can cause paralysis that may prevent breathing, bleeding disorders that can lead to a fatal haemorrhage, irreversible kidney failure and tissue damage that can cause permanent disability and limb amputation.


Agricultural workers and children are the most affected. Children often suffer more severe effects than adults, due to their smaller body mass.


Global situation

Snake bite is a neglected public health issue in many tropical and subtropical countries. About 5.4 million snake bites occur each year, resulting in 1.8 to 2.7 million cases of envenomings (poisoning from snake bites).There are between 81 410 and 137 880 deaths and around three times as many amputations and other permanent disabilities each year.
Most of these occur in Africa, Asia and Latin America. In Asia up to 2 million people are envenomed by snakes each year, while in Africa there are an estimated 435 000 to 580 000 snake bites annually that need treatment. Envenoming affects women, children and farmers in poor rural communities in low- and middle-income countries. The highest burden occurs in countries where health systems are weakest and medical resources sparse.
Bites by venomous snakes can cause acute medical emergencies involving severe paralysis that may prevent breathing, cause bleeding disorders that can lead to fatal haemorrhage, cause irreversible kidney failure and severe local tissue destruction that can cause permanent disability and limb amputation. Children may suffer more severe effects, and can experience the effects more quickly than adults due to their smaller body mass.
In contrast to many other serious health conditions, a highly effective treatment exists. Most deaths and serious consequences from snake bites are entirely preventable by making safe and effective antivenoms more widely available and accessible. High quality snake antivenoms are the only effective treatment to prevent or reverse most of the venomous effects of snake bites. They are included in theWHO List of essential medicines and should be part of any primary health care package where snake bites occur.

Challenges producing antivenoms

A significant challenge in manufacturing of antivenoms is the preparation of the correct immunogens (snake venoms). At present very few countries have capacity to produce snake venoms of adequate quality for antivenom manufacture, and many manufacturers rely on common commercial sources. These may not properly reflect the geographical variation that occurs in the venoms of some widespread species.In addition, lack of regulatory capacity for the control of antivenoms in countries with significant snake bite problems results in an inability to assess the quality and appropriateness of the antivenoms.
A combination of factors has led to the present crisis. Poor data on the number and type of snake bites have led to difficulty in estimating needs, and deficient distribution policies have further contributed to manufacturers reducing or stopping production or increasing the prices of antivenoms. Poor regulation and the marketing of inappropriate or poor quality antivenoms has also resulted in a loss of confidence in some of the available antivenoms by clinicians, health managers, and patients, which has further eroded demand.

Weak health systems and lack of data

Health systems in many countries where snake bites are common often lack the infrastructure and resources to collect robust statistical data on the problem. Assessing the true impact is further complicated by the fact that cases reported to health ministries by clinics and hospitals are often only a small proportion of the actual burden because many victims never reach primary care facilities, and are therefore unreported. This is contributed to by socio-economic and cultural factors that influence treatment-seeking behavior with many victims opting for traditional practices rather than hospital care.
Under-reporting of snake bite incidence and mortality is common. In Nepal, for example, where 90% of the population lives in rural areas, the Ministry of Health reported 480 snake bites resulting in 22 deaths for the year 2000, yet figures for the same year collected in a community based study of one region (Eastern Nepal) detailed 4078 bites and 396 deaths (1). Likewise, a very large community- level study of snakebite deaths in India gave a direct estimate of 45 ,900 (99% CI: 40,900-50,900) deaths in 2005, which is over 30 times higher than the Government of India’s official figure(2). A comparison of hospital-registered deaths in one district of Sri Lanka to data from the Registrar-General’s office on deaths demonstrated that 62.5% of deaths from snakebite envenoming were not reported in hospital data (3).
In situations where data on snakebite envenoming is poor, it is difficult to accurately determine the need for antivenoms. This leads to under-estimation of antivenom needs by national health authorities driving down demand for manufacturers to produce antivenom products, and for some, their departure from the market. Weak regulatory systems in countries that lack expertise in antivenom evaluation can contribute to inappropriate procurement of ineffective or incorrect products. Inefficient distribution strategies can hinder access to antivenoms and create shortages of supply.

Low production of antivenoms

Given low demand, several manufacturers have ceased production, and the price of some antivenom products have dramatically increased in the last 20 years, making treatment unaffordable for the majority of those who need it. Rising prices also further suppress demand, to the extent that antivenom availability has declined significantly or even disappeared in some areas. The entry into some markets of inappropriate, untested, or even fake antivenom products has also undermined confidence in antivenom therapy generally.
Many believe that unless strong and decisive action is taken quickly, antivenom supply failure is imminent in Africa and in some countries in Asia.

WHO response

WHO has taken steps to raise the awareness of health authorities and policy makers on this issue. In December 2015 a programme to evaluate the potential safety and effectiveness of current antivenom products intended for use in sub-Saharan Africa was launched by the WHO. The results of this detailed technical and laboratory assessment will provide procurement agencies with informed guidance on which antivenoms best suit their needs. Following a request by several UN member states, WHO formally listed snakebite envenoming as a highest priority neglected tropical disease in June 2017.
A Snakebite Envenoming Working Group established that same year was tasked with informing the development of a strategic WHO road map on snakebites. This strategy focuses on a 50% reduction in mortality and disability caused by snakebite envenoming by 2030. This aim will be achieved through four key objectives:

Empower and engage communities,


Ensure safe, effective treatment,


Strengthen health systems, and


Increase partnerships, coordination and resources.


A primer on the WHO response was published in the journal PLoS Neglected Tropical Diseases in February 2019 (4). The full strategy will be launched in May 2019 with copies of the road map  available on the WHO website after the launch.
WHO will work closely with a range of partners to ensure the successful implementation of the road map which focuses on activities in countries and regions where snakebite envenoming occurs, supported by technical units from WHO.
WHO urges regulators, producers, researchers, clinicians, national and regional health authorities, and international and community organizations to work together to improve the availability of reliable epidemiological data on snake bites, the regulatory control of antivenoms and their distribution policies.

Law enforcement agencies crack the whip on violators in central zone

Law enforcers have launched a crackdown on two-wheeler riders found violating the helmet rule and motorists not wearing seat belts. Even as checks had been accorded a thrust of late, the police acting on a direction from their headquarters executed a special drive on Saturday and Sunday specifically aimed at checking these two violations.
Positioning themselves at vantage spots, multiple police teams assiduously carried out the directive from the State police headquarters, booking a large number of cases and realising fine amount from the violators in a matter of just two days.
Simultaneous drives carried out across the Central Zone encompassing Tiruchi and seven other districts resulted in the booking of over 34,000 cases, with helmet rule violation cases accounting for the majority.

The special drive was conducted to reinforce to vehicle users the need for strict adherence to helmet rule and wearing seat belt. Ahead of the two -day special drive, police sources say checks had been intensified across Tiruchi Range encompassing Tiruchi, Pudukottai, Karur, Perambalur and Ariyalur districts from late last month. The sheer number of cases booked against two-wheeler riders for not wearing helmet and against those driving car without strapping themselves with seat belt is a pointer to the thrust accorded by the law enforcers.

Karur tops list

Topping the list in Tiruchi Range was Karur where the police had booked over 50,000 cases against violators for not wearing helmet or seat belt from August-end till date. Tiruchi district was placed second followed by Pudukottai, Ariyalur and Perambalur districts respectively.
The enforcement measure over the past few days has had an impact among several two-wheeler riders in Tiruchi City as many have started wearing helmet, says Commissioner of Police, Tiruchi A. Amalraj. The helmet rule compliance is about 80% now, he adds.
As a fall out of the periodic enforcement measures and amid reports of hefty fine to be slapped for not wearing helmet in the wake of the amendment to the Motor Vehicle Act, there seems to be a realisation among two-wheeler riders to adhere to the helmet rule, say field-level police officers.
Although similar intensified drives were conducted in the past in Tiruchi city, it remained for a particular period of time. However, this time around the drives seems to have brought in a perceptible change among two-wheeler riders, say the officers adding that there was no revision in the fine amount collected as of now. The helmet rule is being adhered to even by police personnel riding two-wheelers to set an example for others, say law enforcers.
Notwithstanding booking of cases in large numbers, the impact with respect to compliance was not to the expected level in the rural parts, say police sources.

‘To be intensified’

“We plan to intensify the checks in areas identified as accident-prone in Tiruchi Range” says the Deputy Inspector General of Police, Tiruchi Range V. Balakrishnan.

“Helmet is a protective gear which would prevent grievous injury to the head and reduce mortality,” says Dr. M.A. Aleem -a city-based neurologist.
Alongside enforcement, the official machinery should take steps to infuse road safety culture right from the school level as this help would make the young minds to adhere to traffic rules in the later stages of their life, he said

Vegetarian Diet and Neurological Diseases

People with many different dietary habits call themselves vegetarians. They are

1. Vegans (total pure vegetarians): They do not eat meat, poultry, fish, or any products derived from animals, including eggs, dairy products, and gelatin.

2. Lacto-ovo vegetarians: They do not eat meat, poultry, or fish, but do eat eggs and dairy products.

3. Lacto vegetarians: They eat no meat, poultry, fish, or eggs, but do consume dairy products.

4. Ovo vegetarians: They eat no meat, poultry, fish, or dairy products, but do eat eggs.

5. Partial vegetarians: They avoid meat but may eat fish (pesco-vegetarian, pescatarian) or poultry (pollo-vegetarian).

Compared with non-vegetarian eaters, vegetarians tend to consume less saturated fat cholesterol and more vitamins C E, dietary fiber, folic acid, potassium, magnesium, and phytochemicals such as carotenoids and flavonoids. As a result, they're likely to have lower total and LDL cholesterol, lower blood pressure, and lower body mass index (BMI). These things are associated with longevity and a reduced risk for many chronic neurological diseases.

Vegetarian life style is superior to a non-vegetarian life style from the angle of prevention of disease. Vegetarian diet can also play an important role in the prevention and treatment of many chronic, progressive neurological diseases.

Migraine

Migraine has a very close relationship to the kind of food we eat. A Ttyrosine rich non-vegetarian diet can exacerbate migraine. So the vegetarian diet may be more suitable for a patient with migraine.

Stroke

The main risk factors for stroke are hypertension diabetes and hypercholesterolaemia. Vegetarian diet is beneficial in the treatment and prevention of hypertension, diabetes mellitus, hypercholesterolaemia, and obesity. Vegetarians are also less inclined to use tobacco and misuse alcohol. So the vegetarian diet may be beneficial for the prevention of stroke.

A recent study found that there is no difference between meat eaters and vegetarians in the risk of ischaemic stroke. But that study found that the vegetarians had a 43% increased risk of haemorrhagic stroke.

Other Neurological Diseases

Neurological diseases such as dementia, multiple sclerosis, motor neurone disease, peripheral neuropathies, Parkinson's disease and other chronic progressive neurodegenerative diseases of the brain and spinal cord may show benefits with fiber, essential fatty acids, minerals, trace elements and vitamin rich vegetarian diets than with non-vegetarian diets.

Vegetarian diet can also help to boost the human immune system, and this can also be useful to prevent neuroinfections and neuroinflammatary disorders.

Memory and Intellectual capacity

There is a long list of intellectuals who are vegetarian who have made an incredible mark in their life time. The list includes Isaac Newton, George Bernard Shaw, Shelley, Milton, and Voltaire. In India, Mahatma Gandhi, Rajendra Prasad, Rajagopalachari, Lal Bahadur Shastri, Naranda Modi and many more are vegetarians. Vegetarianism will increase memory and intellectual activity of a very high order. In my county Mahavira, Buddha and Saints of Hinduism all advocated vegetarianism.

Like intellectual activity, even physical functions seem to be better preserved in vegetarians as compared to non-vegetarians.

In recent days plant-based eating is recognized as not only nutritionally sufficient but also as a way to reduce the risk for many chronic lifestyle diseases.

Now more and more people are turning to vegetarianism and veganism, which is good news for the fight against climate change also.

I am having the research experience from 1997 to till date as a DM Post graduate student , as an Asst. Professor, Professor and as Emeritus Professor of Neurology. I have 44 Publications and 47 presentations of my research works in my credit. As a DM Post graduate M.A .ALEEM’S is research work at Madurai Medical College, Madurai (MMC) has published 11 research papers and it has 23 citations (https://www.researchgate.net/scientific-contributions/5855891_M_A_Aleem).As an Assistant Professor of neurology my research works were accepted by the World Federation of Neurology and I have presented my papers on 1.Seizure related injuries in rural epileptic women from in southern India in the World Congress of Neurology (WCN) 2001 London. 2.Iatrogenic Seizures – A clinical study from southern India in WCN 2001London.3.Circardian onset of stroke in young rural southern Indian population in WCN 2005Sydney 4.Study of psychogenic non epileptic Seizure in southern Indian rural population at WCN 2009 at Bangkok in Thailand. 5.Diffusion MR imaging in status epileptics – Reversibility a study from southern India at WCN 2015 Santiago Chile. All this above Presentation are Published in journal of Neurological science in above in that years as abstract. My research works was accepted by the International League Against Epilepsy and I have presented the paper on Cerebral calcification and epilepsy in rural population at 25thInternational Epilepsy Congress(IEC) 2003 Lisbon Portugal and a paper on southern Indian study on Iatrogenic seizure in 26th IEC 2005 Paris. Both were published as abstract in that year Epilepsia Journal. I have also presented my neurological research work in IANCON, and Neurocon.I have grant from Government of Tamilnadu state research committee to do the research work oncircadian rythem and stroke in southern Indian rural population for one year in 2004.I am a guide for Neurology research work with EEG & fMRI at Department of instrumentation and control. Engineering at National institute of Technology Trichy for PhD scholar from 2013 to 2016.

I am a Life member in IAN since 1999 (L M 431).Served in IAN as a National Executive Committee member (2002 – 2009). I was appointed by IAN as organising Secretary to conduct the 2010IANCON for first thetime in 2nd tier city in Trichy As per IAN Instruction I have organised a preconference workshop at during IANCON 2010at Trichy. I am also served as member in the organising committee of IANCON - 2010Madurai and IANCON 2003 VIZAK and IANCON 2004 Nagpur .I have taken part in all IAN instructed Indian National Brain week activities, worldBrain day activities and other Neurological awareness activities.I have contributed articles for Reviews in Neurology on Myasthenic syndrome and on cerebral malaria in 2000 IANCON, and an Hypertension management in stroke in 2003 IANCON.I have chaired sessions and presented papers in IANCON many times. Course at Thirunelveli Govt medical college in 2007. He has published 11 books in regional language on Neurology topics to create awareness among people. He is the examiner in the Tamilnadu Dr. MGR medical University. At present he is the president for Trichy Neuro club and Secretary for Neurologist association of Tiruchirappalli. He is the founder of NAT at Trichy 2017.He is a also instrumental in bringing the International Epilepsy Day by ILAE/IBE in 2015.Prof Dr. M.A. Aleem is conferred with best doctor award in the field of neurology by the Tamilnadu Dr. MGR Medical University in 2012.He had junior travelling fellowship award from WFN to attend WCN 2001 at London. He is graduated from Madras University in 1980. He has received his MD., General medicine from Bharadhidasan University in 1986 and his DM,Neurology through the Tamilnadu Dr.M.G.M Medical University in 1999.ln 2001 as IAN sponsored candidate he has undergone training at University college of London' Institute of Neurology queen' s Square London in the field of epilepsy . He was the visiting professor for Annamalai University's Raja Muthaiya Chettiar Medical college and hospital chidambram. A visiting consultant Neurologist at Ordinance factory Govt of India Trichy . for 16 years from 2004 He is deeply interested in medical education and services since 1986. He has orated, presented and published more than 107 research papers in neurology and General medicine at various National and International, conference and Journals.

Now he is the Emeritus Professor at the Tamilnadu Dr.MGR Medical University of Government of Tamilnadu

He is also the senior consultant neurologist and epileptologist at ABC Hospital Trichy He is a visiting professor for Dhanalakhmi Srinivasan medical college and hospital perambalur since 2016.He is the visiting neurology Consultant for Apollo SpecialityHospital Trichy since 2016.

By creating this awareness time between the onset of neurological symptoms and seeking of medical help has been drastically reduced in our rural population residing in the central part of Tamil Nadu
