COVID year of 2020 in Trichy 

Trichy has recorded the first COVID-19 case in March 27,2020. The first COVID-19 related death was reported in  June 3,2020.

In this COVID-19 Pandemic year 2020 Trichy has celebrated the Ramzan , Bakrid, Diwali, Christmas and Vaikunta Ekadasi including Paramapada vasal opening without any minor or major Outbreak of news cases of COVID-19. Thank God , people and the administrators of Trichy city and Trichy District for their support by all means to prevent the new COVID-19 Outbreak. 

As of now on 52nd week of this Pandemic Trichy has recorded very low death rate of 1.22% and the total confirmed COVID-19 cases were 14,070 on Saturday (26.12.20).
There is a steady declining in the number of new COVID-19 cases and Hospital admissions. These are the  good signs of COVID-19 control in the Trichy city and in the district.

Now our district is getting ready  by requesting the Medical institutions and hospitals to upload the beneficiaries in COVID-19 vaccination portal.

In 52nd week of the COVID-19 Pandemic, world is preparing to face many challenges coming out of the newer COVID-19 variant and vaccination

On 14 December 2020, authorities of the United Kingdom (UK) reported to WHO that a new SARS-CoV-2 (COVID-19) variant was identified through viral genomic sequencing.

Earlier Analysis indicates that the variant may spread more readily between people. Investigations are ongoing to determine if this variant is associated with any changes in the severity of symptoms, antibody response or vaccine efficacy.

Preliminary reports by the UK are that this variant is more transmissible than previous circulating viruses, with an estimated increase of between 40% and 70% in transmissibility (adding 0.4 to the basic reproduction number R0, bringing it to a range of 1.5 to 1.7).



Other than UK the new COVID-19 variant has been identified in several countries including Australia, Denmark, Italy, Iceland South Africa Nigeria and in
Netherlands.

The new COVID-19 strain is not detected in India as of on 25.12.2020.

India has noted several mutations in the novel coronavirus since March, out of which one was a super-spreader but it died on its own, said a top official from the Institute of Genomics and Integrative Biology (IGIB).

World has also facing a few challenges towards vaccination for COVID-19.

As positive results emerge at last with vaccination against COVID-19 , researchers must help the world to address the vaccine hesitancy, supply logistics and pricing to vaccinate every one in developed, developing and in underdeveloped countries

Common side effects on the arm where you got the shot include pain and swelling. You could also experience fever, chills, tiredness and headache. Some time a few allergic reactions has also been reported in a few with COVID-19 vaccination

Scientists say there’s no evidence that vaccines currently being deployed in the UK - made by Pfizer and BioNtech - or other COVID-19 shots in development will not protect against this new COVID-19 variant .

Prof Dr.Aleem M.D.,D.M., (Neuro) a rural Neurologist / Neuroscientist was the Former Dean I/C vice principal HOD& professor of Neurology KAPV Government Medical college& MGM Government hospital Trichy. He is graduated from Madras University in 1980. He has received his MD., General medicine from Bharadhidasan University in 1986 and his DM,Neurology through the Tamilnadu Dr.M.G.M Medical University in 1999.ln 2001 as IAN sponsored candidate he has undergone training at University college of London' Institute of Neurology queen' s Square London in the field of epilepsy . He was the visiting professor for Annamalai University's Raja Muthaiya Chettiar Medical college and hospital chidambram. A visiting consultant Neurologist at Ordinance factory Govt of India Trichy . for 16 years from 2004 He is deeply interested in medical education and services since 1986. He has orated, presented and published more than 107 research papers in neurology and General medicine at various National and International, conference and Journals.

Now he is the Emeritus Professor at the Tamilnadu Dr.MGR Medical University of Government of Tamilnadu

He is also the senior consultant neurologist and epileptologist at ABC Hospital Trichy He is a visiting professor for Dhanalakhmi Srinivasan medical college and hospital perambalur since 2016.He is the visiting neurology Consultant for Apollo SpecialityHospital Trichy since 2016.

By creating this awareness time between the onset of neurological symptoms and seeking of medical help has been drastically reduced in our rural population residing in the central part of Tamil Nadu

Life member in IAN since 1999. Dr. Aleem has served as National executive member for IAN. He was the organizing secretary for 18th IANCON Trichy in 2010. He was also a joint Organizing secretary for 2000 IANCON at Madurai. Dr. Aleem has contributed articles for the reviews in Neurology released by IAN and he has chaired sessions in IANCON. Awarded Fellow of Indian Academy of Neurology in 2019

Prof Dr. Aleem is a life member in TAN. He has talked and chaired session in TANCON. He has organised TANCON at Trichy in 2017. He was in various post in TAN since 2016. President of Tamilnadu Pondicherry Association of neurologists for me year2017-2018 and ha has given the president at TANCON 2018at courtallam Tamilnadu now he is part president of TAN for year2018-2020.

Life member since 1999.Organizing committee member for the Neurocon 2006 at Madurai and delivered lecture in Neurcon CME and chaired Sessions in Neurocon. He has served as an executive committee member in NSI and contributed articles for the CME book

Contribution to Neuroscience:

Prof Dr. Aleem has created and established the department of neurology at KAPV GOVT medical college and MGM GOVT hospital Trichy. He had a role in starting the DM Neurology higher specialty. Course at Thirunelveli Govt medical college in 2007. He has published11 books in regional language on Neurology topics to create awareness among people. He is the examiner in the Tamilnadu Dr. MGR medical University. At present he is the president for Trichy Neuro club and Secretary for Neurologist association of Tiruchirappalli. He is the founder of NAT at Trichy 2017.He is a also instrumental in bringing the International Epilepsy Day by ILAE/IBE in 2015. He has instituted award in (IAN ) Indian Academy of Neurology in the name of Neurologist Association of Tamilnadu Pondicherry From 2018.

He served as

MCI Nodal officer from 2008-2009and he served as a vigilance officer at AGM Govt hospital of KAPVGMC in 2009-2010 and worked as an administrative officer at AGM Govt Hospital of KAPVGMC in 2009-2010. He was medical superintendent incharge for AGM Govt Hospital Trichy in 2011 and Dean incharge for Trichy KAPVGMC in 2009 . He served as vice Principal of Trichy KAPVGMC from 2010 till 2014. He in retired as vice Principal, HOD Professor of Neurology of31.12.2014 from KAPVGMC Trichy.

Prof Dr. M.A. Aleem is conferred with best doctor award in the field of neurology by the Tamilnadu Dr. MGR Medical University in 2012.He had junior travelling fellowship award from WFN to attend WCN 2001 at London. He had congress attendee award from ILAE to attend 25th IEC at Lisbon. He had travelling bursary award from ILAE to attend 6th AOEC at Malaysia in 2006. He had commendation certificate from Trichy District collector for this Neurological Health care services for the year. 2005, 20062014 – 2015, 2015 – 2016, 2016 – 2017.

Prof Dr. Aleem had Republic day Best Doctor award for his contribution to rural Neurological awareness activity from Trichy collector in 2010. He had best administrator award from the Trichy district collect in 2013.

He also had Dr. Banumathy Muruganathan award for the excellence in medical publication in Tamil by Tamilnadu IMA in 2008.He was nominated as Epilepsy purple day Ambassador in 2012 by EANS Newyork.

He was nominated as Trichy district welfare fund committee life member by district collector and Truchy DWFC committee from 2017. He is also an advisor to phd students at NITT Trichy in the Instrumentation Engineering department.

Dr Aleem was awarded 1st prize for his suggestions of Trichy SMART CITY by the Tiruchirappalli city Corporation in 2015

Dr Aleem was awarded for his our Ptime Minister Modi ' s SWACHH BHARAT activities by Tiruchirappalli district collector in 2017.

He is awarded with KALAM Award for Quality of life improvement by South Zone Board if Continuing Education Ozone on 18.11.2018.

I am having the research experience from 1997 to till date as a DM Post graduate student , as an Asst. Professor, Professor and as Emeritus Professor of Neurology. I have 44 Publications and 47 presentations of my research works in my credit. As a DM Post graduate M.A .ALEEM’S is research work at Madurai Medical College, Madurai (MMC) has published 11 research papers and it has 23 citations (https://www.researchgate.net/scientific-contributions/5855891_M_A_Aleem).As an Assistant Professor of neurology my research works were accepted by the World Federation of Neurology and I have presented my papers on 1.Seizure related injuries in rural epileptic women from in southern India in the World Congress of Neurology (WCN) 2001 London. 2.Iatrogenic Seizures – A clinical study from southern India in WCN 2001London.3.Circardian onset of stroke in young rural southern Indian population in WCN 2005Sydney 4.Study of psychogenic non epileptic Seizure in southern Indian rural population at WCN 2009 at Bangkok in Thailand. 5.Diffusion MR imaging in status epileptics – Reversibility a study from southern India at WCN 2015 Santiago Chile. Course at Thirunelveli Govt medical college in 2007. He has published 11 books in regional language on Neurology topics to create awareness among people. He is the examiner in the Tamilnadu Dr. MGR medical University. At present he is the president for Trichy Neuro club and Secretary for Neurologist association of Tiruchirappalli. He is the founder of NAT at Trichy 2017.He is a also instrumental in bringing the International Epilepsy Day by ILAE/IBE in 2015.Prof Dr. M.A. Aleem is conferred with best doctor award in the field of neurology by the Tamilnadu Dr. MGR Medical University in 2012.He had junior travelling fellowship award from WFN to attend WCN 2001 at London. SARS-CoV-2 Variant (COVID-19 Variant ?COVID-20v1 ) WHO Guidance - M.A.Aleem Neurologist #Trichy #Tamilnadu #India

(COVID-19 Variant)



On 14 December 2020, authorities of the United Kingdom (UK) reported to WHO that a new SARS-CoV-2 (COVID-19) variant was identified through viral genomic sequencing. 

This variant is referred to as SARS-CoV-2 VUI 202012/01 (Variant Under Investigation, year 2020, month 12, variant 01).


 Initial analysis indicates that the variant may spread more readily between people. Investigations are ongoing to determine if this variant is associated with any changes in the severity of symptoms, antibody response or vaccine efficacy.

A total of 1108 cases infected with SARS-CoV-2 VUI 202012/01 have been detected in the UK as of 13 December 2020. 

The variant was picked up as part of an epidemiological and virological investigation initiated earlier in December 2020 following an unexpected rise in COVID-19 cases in South East England. It was characterized by a more than three-fold increase in the 14-day case notification rate from epidemiological week 41 to week 50 (5 October to 13 December 2020). 

On average, between 5 - 10% of all SARS-CoV-2 viruses have routinely been sequenced in the UK and 4% routinely sequenced in South East England since the beginning of the pandemic. 

From 5 October to 13 December, over 50% of isolates were identified as the variant strain in South East England. Retrospective analysis traced the first identified variant to Kent, South East England, on 20 September 2020, which was followed by a rapid increase of the same variant identified later in November. Most COVID-19 cases from whom this variant has been identified have occurred in people under 60 years of age.

The variant is defined by the presence of a range of 14 mutations resulting in amino acid changes and three deletions. Some of these mutations may influence the transmissibility of the virus in humans:

One of the mutations identified (N501Y) is altering an amino acid within the six key residues in the receptor binding domain (RBD). According to the Global Initiative on Sharing Avian Influenza Data (GISAID) database, this same receptor binding domain mutation (N501Y) has been independently reported in several countries including South Africa (n=45) and Australia (n=37). Sequence analysis revealed that N501Y mutation of the virus reported in the UK and South Africa originated separately.

Another mutation of biological significance, P681H, has been found in the RBD.
Finally, the deletion at position 69/70 has been found to affect the performance of some diagnostic PCR assays that use an S gene target. Most PCR assays worldwide use multiple targets and therefore the impact of the variant on diagnostics is not anticipated to be significant.


The new VUI-202012/01 variant has been identified in several countries including Australia, Denmark, Italy, Iceland and the Netherlands.


Preliminary reports by the UK are that this variant is more transmissible than previous circulating viruses, with an estimated increase of between 40% and 70% in transmissibility (adding 0.4 to the basic reproduction number R0, bringing it to a range of 1.5 to 1.7).

 Laboratory studies are ongoing to determine whether these variant viruses have different biological properties or alter vaccine efficacy. There is not enough information at present to determine if this variant is associated with any change in severity of clinical disease, antibody response or vaccine efficacy.

Public health response

The authorities in the UK are conducting epidemiological and virological investigations to further assess the transmissibility, infection-severity, risk of reinfection and antibody response of this new variant. As one of the mutations (N501Y) is in the receptor binding domain, the authorities are urgently investigating the neutralization activity of sera from recovered and vaccinated patients against this variant to determine if there is any impact on vaccine performance.

Genomic data of this variant has been uploaded to GISAID by the authorities of the UK and genomic surveillance of the virus continues across the country to monitor the situation.

On 19 December 2020, authorities in the UK announced that affected areas would be subject to Tier 4 restrictions including reduced social gatherings, tighter movement restrictions, requests to work from home wherever possible, and closures of non-essential businesses.

WHO risk assessment

All viruses, including SARS-CoV-2, change over time, but most of these mutations or changes do not have a direct benefit to the virus or may even be detrimental to its propagation. Further laboratory investigations are required to more fully understand the impact of specific mutation on viral properties and the effectiveness of diagnostics, therapeutics and vaccines. These investigations are complex and require time and collaboration amongst different research groups.

The sharing of full genome sequences is facilitating detailed analyses by partners. The WHO SARS-CoV-2 Virus Evolution Working Group is working with colleagues from the UK to better understand the available results and support further studies.

Further epidemiological and laboratory-based studies are rapidly required to understand the implications of these viruses in terms of available SARS-CoV-2 clinical presentation, diagnosis, treatment and vaccine development.

WHO advice


The preliminary findings by the UK signal the broader issue of SARS-CoV-2 virus mutations, and WHO underscores the importance of prompt sharing of epidemiological, virological and full genome sequence information with other countries and research teams, including through open-source platforms such as GISAID and others.

WHO advises that further epidemiological and virological studies be conducted to understand the specific mutations described by the UK and other countries to further investigate any changes in the function of the virus in terms of infectivity and pathogenicity. WHO advises all countries to increase the routine sequencing of SARS-CoV-2 viruses where possible, and sharing of sequence data internationally, in particular, to report if the same mutations of concern are found.

WHO would like to draw attention to the concern about the reported loss of performance of PCR assays that target the spike (S) gene of the virus. Laboratories using commercial PCR kits for which the targeted viral genes are not clearly identified in the manufacturer’s instructions are advised to contact the manufacturer for more information. Laboratories using in-house PCR assays that target the S gene of the virus should also be aware of this potential issue. In order to limit the impact on the detection capacities in the countries, an approach using different assays in parallel or multiplex assays targeting different viral genes is also recommended to allow the detection of potential arising variants.

All countries need to assess their level of local transmission and apply appropriate prevention and control activities including adapting public health and social measures as per WHO guidance.

It is important to remind communities and health workers of the basic principles to reduce the general risk of transmission of acute respiratory infections:

Avoiding close contact with people suffering from acute respiratory infections;
Frequent hand-washing, especially after direct contact with ill people or their environment;
People with symptoms of acute respiratory infection should practice cough etiquette (maintain distance, cover coughs and sneezes with disposable tissues or clothing, and wash hands);
Within healthcare facilities, enhance standard infection prevention and control practices in hospitals, especially in emergency departments; and
Wearing masks where appropriate and ensuring good ventilation.
WHO recommends the health measures as listed above for all travelers, including to and from the UK. In case of symptoms suggestive of acute respiratory illness either during or after travel, travelers are encouraged to seek medical attention and share their travel history with their health care provider. Health authorities should work with travel, transport, and tourism sectors to provide travelers with information to reduce the general risk of acute respiratory infections, via travel health clinics, travel agencies, conveyance operators, and at points of entry.

In line with the advice provided by the Emergency Committee on COVID-19 at its most recent meeting, WHO recommends that States Parties should regularly re-consider measures applied to international travel in compliance with Article 43 of the IHR (2005) and continue to provide information and rationale to WHO on measures that significantly interfere with international traffic. Countries should also ensure that measures affecting international traffic are risk-based, evidence-based, coherent, proportionate and time limited.

WHO has recently published an interim guidance – "Considerations for implementing a risk-based approach to international travel in the context of COVID-19", to provide countries with a risk based approach to decision-making process for calibrating travel related risk mitigation measures in the context of international travel, aiming at reducing travel-associated exportation, importation and onward transmission of SARS-CoV-2 while avoiding unnecessary interference with international traffic.

WHO recommends that countries take a risk-based approach. National authorities are encouraged to publish their risk assessment methodology and the list of departure countries to which restrictions apply; and these should be updated regularly. In all circumstances, essential travel (e.g. emergency responders; providers of public health technical support; critical personnel in transport and security sector such as seafarers; repatriations; and cargo transport for essential supplies such as food, medicines, and fuel) identified by countries should always be prioritized and facilitated.

As of now on 23.12 20 India has not reported any such COVID-19 variant in any states.  India has taken taken various preventive steps including cancellation of flight from UK till December 31st, 2020 to check the spread of new COVID-19 Variant in India.

India has lower COVID-19 Deaths

With the leadership of our Prime Minister Modi, in the 51st week of the COVID-19 Pandemic, India has a lower COVID-19 related death rate of 1.45% and a recovery rate of 95.35%.
The daily COVID-19 cases numbers have come down to around 18k.

With masks, social distancing and personal hygiene and without any vaccines, India has achieved and continues saving lives and livelihoods with Our PM Modi's leadership.

So far Two COVID-19 vaccines are in use in the world in the 51st week of the COVID-19 Pandemic

The FDA has approved two USA vaccines for COVID-19.

The first one is the Pfizer vaccine, which has 95% efficacy with two doses in a 21 day interval. This should be stored at -60°to -40°c. This vaccine is indicated in the age group of 16 and above.

The second vaccine is Moderna, which has 94.5% efficacy in two doses in a 28 day interval. This should be stored at -25° to -15°c. This vaccine is advised in the age group of 18 and above.

Pfizer and Moderna are mRNA vaccines. Messenger RNA molecules of the virus code for parts of the target pathogen that are recognised by our immune system (antigens). Inside our body's cells, the RNA molecules are converted into antigens, which are then detected by our immune cells to produce antibodies.

With and without vaccines, the wearing of masks, washing of hands and keeping of social distancing are the corner stones to beat the COVID-19 during this Pandemic.

COVID-19: India continues saving lives and livelihoods

At present there are about 0.9% of the world population is infected with COVID-19 in the 50th week of the current COVID-19 Pandemic.

As of on 11.12.2020 there are more than
70.8 million (70,848,102) confirmed COVID-19 cases with over 1.5 million (1,591,266) deaths in the 218 countries (Samoa is the 218th country added on 19.11
20 )and territories in all continents and in 2 International conveyances in the world.

The global total COVID-19 cases has crossed 70.8 million mark
and it has registered a global ever highest single day spike of over 6.7 lakh ( 676,806) cases with the single day highest number of deaths of 12,625 cases on 10.12.20.

Among the currently COVID-19 infected people , mildly infected people has been raised to 99.5% and the critically affected people has come down to 0.5% on 9.12.20 during this Pandemic .

USA has registered a higher single day spike of over 2 lakh (217,779) COVID-19 cases and the total cases has crossed 16 million mark (16,039,393) with over 2.9 lakh (299,692) deaths ( single day deaths of 2974) on 10.12.20

Cases and death rate in India including in the state of Tamilnadu and in my own district of Trichy are declining .
India is in the second place after USA in the list of global single day spike of COVID-19 cases with the number of over 34k (34,666) with the single day death of 487 cases and the totol has crossed 9.7 million mark (9,796,992)
on 10.12.2020 during this Pandemic.

In India the recovered cases were over 9.2 million mark (9,290,188) in India on 10.12.2020

India continues saving lives and livelihoods – recording more COVID-19 recoveries while lifting restrictions.

A decrease in new weekly COVID-19 cases is reported in India in the 50th week of the Pandemic.

COVID-19 situations in the 1st day of 50th of COVID-19 Pandemic

The global total COVID-19 cases has crossed 67.3 million mark (67,377,122)
and it has registered a global higher single day spike of  over 5.3 lakh ( 534,677) cases with the single day higher number of deaths   of 7,534 cases on 6.12.20






USA  has registered a higher single day spike  of  over 1.7 lakh number of 173,861 COVID-19  cases and the total cases has crossed 15.1 million mark (15,159,529) with over 2.8 lakh (288,906) deaths ( single day  deaths of higher 1076) on 6.12.20



Cases in India and in Tamilnadu Trichy are declining and India is in the second place after USA in the list of global single day spike of  COVID-19 cases with the number  of over 32k (32,272)  with the single  day death of 374 cases and the totol has crossed 9.6 million mark (9,676,801)
 on 6.12.2020 during this Pandemic. The recovered cases were over 9.1 million mark (9,138,171) in India on 6.12.2020







As of on 7.12. 20 in 218 countries (Samoa is the 218th country added on 19.11
20 )and territories in all continents and in 2 International conveyances  over 67.3 Million mark (67,385,185) with India over 9.6 Million mark (9,676,801) and 790,240
 in the State of Tamilnadu , and in Trichy 13,590 cases were reported with COVID-19 infection .Recovered cases were over 46.5 Million mark (46,578,897) and over 1.5 million (1,541,638) deaths in world  which include   140,590 in India and 11,793 in Tamilnadu  and 172 at Trichy has been recorded as of  7.12.2020.

Post-Deepavali, no significant spike in fresh cases in Tiruchi: Collector

Post-Deepavali, there has been no significant spike in fresh cases of COVID-19 in Tiruchi district.

Though the cases were on a declining trend in the district for the last three to four weeks, it was feared that the number of fresh cases may go up post-Deepavali due to festivities, mutual visits of people and large-scale rush of shoppers on commercial streets of the city for festival purchase

However, statistics of the Health Department shows that no area in the district has registered abnormal rise in fresh case of COVID-19 since November 14. Fresh cases are on the steady decline in spite of testing for a high number of samples.

The COVID-19 testing laboratory at K.A.P. Viswanatham Medical College receives samples from about 80 points of sample collections daily including Primary Health Centres, Urban Primary Health Centres, fever outreach camps, surveillance camps and mobile camps.

On an average, the lab receives 1,800 samples a day. However, the daily count of fresh cases hovers around 20 to 30. No cluster of fresh cases have been noticed. A total of 52,816 samples were tested in November. Of them, 916 patients tested positive.

“We have not come across any noticeable spike in any part of the district after Deepavali. Although the fight against COVID-19 is far from over, it is a positive development”” said Collector S. Sivarasu.

As on Monday, 160 patients are under treatment including those of home isolation. Out of 13,421 total cases, 13,089 patients have been discharged from hospitals in the district.

“It has been noted that the viral infection has come down to a great extent. The arrival of newly infected people from other districts has also come down. However, it is too early to come to a conclusion on the spread of COVID-19,” said A. Aravind Kumar, District Epidemiologist.

“There were days when dedicated beds for COVID-19 in private hospitals were overflowing. Now, more than 75% of the beds are vacant. It is a good sign in controlling the virus”” says M.A. Aleem, a city-based neurologist.

There can be no complacency. The next three tofour weeks are crucial. Hence, health, civic and rural development officials have been asked to ramp up surveillance, Mr. Sivarasu said.
