The biggest differences lie in the prefrontal cortex – a part of the brain associated with reasoning – and in the networks of brain cells that link the cortex to regions of the brain that are less about reasoning and thinking and more about emotion.
These circuits – which include dopamine-containing neurons in the prefrontal cortex and deeper areas of the brain, such as the nucleus accumbens and amygdala – may provide a hormonal jolt that causes to embrace foolish behaviours.


 How much of the immaturity  related to the prefrontal cortex and how much is  foolishness, I just don’t know.

Current COVID-19 Status in India

As of now on 27.03.2021 the total COVID-19 cases in India has crossed 11.9 (11,908,910) and active cases number has increased to 452,647. The recovered COVID-19 cases are 94.85% and the death rate is 1.35%.

Till 26.03.2021 India has vaccinated 58,109,773 individuals above the age of 45 with comorbidities.

As far as COVID-19 testing is concerned India has done 239,769,553 million total samples for COVID-19 through 1216 Government and 1154 private labs till 26.03 2021

கால்-கை வலிப்பு எனும் நரம்பியல் பிரச்சினையால் உலக அளவில் 5 கோடிக்கும் அதிகமான மக்களும், இந்தியாவில் 60 லட்சத்துக்கும் அதிகமான மக்களும் பாதிக்கப்பட்டுள்ளனர் என்று மருத்துவ ஆய்வறிக்கை கூறுகிறது.

இன்று உலக வலிப்பு விழிப்புணர்வு நாள். கரோனா பெருந்தொற்றுக் காலகட்டத்தில் கால்-கை வலிப்பு உள்ளவர்களுக்கு என்ன தாக்கங்கள் ஏற்படும் என்பது குறித்து திருச்சியைச் சேர்ந்த மூளை, நரம்பியல் மற்றும் வலிப்பு நோய் நிபுணரும், திருச்சி அரசு மருத்துவக் கல்லூரி மூளை, நரம்பியல் துறையின் முன்னாள் தலைவருமான டாக்டர் எம்.ஏ.அலீமிடம் கேட்டோம்.



வலிப்பு என்பது நோயா? யாருக்கெல்லாம் பாதிப்பு வரும்? குழந்தைகளுக்கும் வருமா?

உலகில் ஆயிரம் பேரில் 4 முதல் 10 பேருக்கு வலிப்பு இருக்கிறது. வலிப்பு என்பது நோயல்ல. நோயின் அறிகுறி. சிலருக்கு மூளையில் உள்ள திசுக்கள் நேரடியாக பாதிக்கப்படுவதால் வலிப்பு ஏற்படலாம். இன்னும் சிலருக்கு உடல்நிலை பாதிக்கப்படும்போது வலிப்பு ஏற்படலாம். மூளையில் ஏற்படக்கூடிய கட்டிகள், மூளைத் திசுக்களைத் தாக்கும் கிருமிகள், மூளைக் காயங்கள், மூளைத் திசுக்கள் சிதைவதால் ஏற்படும் பாதிப்புகள், மூளை ரத்தக்குழாய் பாதிப்பு போன்றவற்றால் வலிப்பு ஏற்படலாம். மதுப் பழக்கம் மூலம் ஏற்படும் தலைக்காயங்கள், மூளையில் ஏற்படும் ரத்தக்குழாய் மாற்றங்களாலும் வலிப்பு ஏற்படலாம்.

பிறவியிலேயே மூளையில் பிரச்சினை உள்ள குழந்தைகள், மூளை வளர்ச்சி குன்றிய குழந்தைகளும்கூட வலிப்பு பாதிப்புக்கு ஆளாகலாம். பிறந்ததிலிருந்து 2 வயதுக்குள் உள்ள குழந்தைகளுக்குக் கிருமித் தொற்று மூளையைத் தாக்கும் ஆபத்து உண்டு. இப்படிப்பட்ட குழந்தைகளுக்கு வலிப்பு வரும். குழந்தையின் ரத்தத்தில் சர்க்கரை, கால்சியம், மக்னீசியம், ‘பிரீடாக்சின்’ போன்ற சத்துக்குறைவு காரணமாகவும் மூளை வளர்ச்சியில் பாதிப்பு ஏற்பட்டு வலிப்பு பாதிப்பில் முடியலாம்.

சில குழந்தைகளுக்குக் காய்ச்சல் வரும்போது வலிப்பு வருவதைப் பார்த்திருப்பீர்கள். பெரும்பாலும் ஆறு மாதம் முதல் ஐந்து வயதுக்கு உட்பட்ட குழந்தைகளுக்குக் காய்ச்சல் காரணமாக வலிப்பு ஏற்படுகிறது. இதை ‘பெப்ரைல் பிட்ஸ்’ என்று சொல்வார்கள். 100 டிகிரி ஃபாரன்ஹீட்டுக்கு அதிகமான காய்ச்சல் குழந்தைகளைத் தாக்கும்போது வலிப்பு ஏற்படலாம். காய்ச்சல் காரணமாக வரும் வலிப்பு சில நிமிடங்களில் மட்டுப்பட்டுவிடும்.


மருத்துவர் அலீம்
கால்- கை வலிப்பு உள்ளவர்களுக்கு கோவிட்-19 பாதிப்புக்கான அதிக ஆபத்துள்ளதா?

பொதுவாக பொதுமக்களோடு ஒப்பிடும்போது கால்-கை வலிப்பு உள்ளவர்களுக்கு தொற்றுக்கான அதிக ஆபத்து இருப்பதற்கான சான்று எதுவும் தற்போது இல்லை. வேறு ஏதாவது சுகாதாரப் பிரச்சினை இல்லாமல் மருந்து எடுப்பதால் வலிப்பு வராதவர்கள் அல்லது எப்போதாவது வலிப்பால் பாதிக்கப்படுகிறவர்களுக்கு அதிக ஆபத்து இல்லை.

கால்-கை வலிப்பு உள்ளவர்களுக்கு கோவிட்-19ஆல் சிக்கல் அதிகமாகும் ஆபத்துள்ளதா?

செயலற்ற நோய்த்தடுப்பு மண்டலம் கொண்டவர்கள் அல்லது தற்போது மருத்துவப் பிரச்சினைகளால் பாதிக்கப்பட்டுள்ளவர்களுக்குச் சிக்கல் ஏற்படும் அதிக ஆபத்து உள்ளது. யுஎஸ் நோய் கட்டுப்பாடு மற்றும் தடுப்பு மையங்கள் (சிடிசி) நீடித்த நரம்புக் கோளாறாக இருப்பதால் கால்-கை வலிப்பைக் கடுமையான கோவிட் தொற்றை அதிகரிக்கும் கோளாறுகளின் பட்டியலில் சேர்த்துள்ளது.

கால்-கை வலிப்புள்ள ஒருவருக்கு கோவிட்- 19 தொற்று ஏற்பட்டால் வலிப்புநோய் மோசமான நிலையை அடையுமா அல்லது இன்னும் அடிக்கடி ஏற்படுமா?

பெரும்பாலான வலிப்பு நோய் உள்ளவர்களுக்கு கோவிட் தொற்று ஏற்பட்டால் வலிப்புகள் மோசமான நிலையை அடையும் ஆபத்து அதிகரிக்கும் என இதுவரை எந்தத் தகவலும் கூறவில்லை.

எனக்கு கரோனா வைரஸ் தொற்று ஏற்பட்டால் அது கால்-கை வலிப்பைத் தூண்டுமா?

கால்-கை வலிப்புள்ள சிலருக்கு ஒரு நோய் இருந்தால் குறிப்பாக அதிக காய்ச்சல் இருந்தால் வலிப்பு ஏற்பட அதிக வாய்ப்பு உண்டு. கரோனா வைரஸுக்குக் காய்ச்சல் ஒரு அறிகுறியாகும். ஆகவே, கால்-கை வலிப்புள்ள சிலருக்கு இது வலிப்பைத் தூண்டலாம்.



கால்-கை வலிப்புள்ளவர்களுக்கு முகக்கவசம் பாதுகாப்பானதா?

கால்-கை வலிப்புள்ளவர்களில் பெரும்பாலானருக்குத் துணி முகக்கவசம் அணிவது பாதுகாப்பானதே. சுவாசிக்கத் தக்க பொருளாலான முகக்கவசத்தை ஒருவர் வலிப்பின்போது அணிந்திருந்தாலும் எந்தத் தீங்கும் ஏற்படாது. முகக்கவசம் தங்களை மிகையாக வெப்பப்படுத்தி சுவாசிக்க முடியாமல் செய்வதனால் கவலைப்படுவதாக சிலர் எங்களிடம் கூறி இருக்கிறார்கள். பொதுமக்களுக்குப் பரிந்துரைக்கப்படும் சுவாசிக்கும் பொருட்களால் தயாரிக்கப்படட் முகக்கவச வகையால் இத்தகைய பிரச்சினை ஏற்பட்டதற்கான சான்று எதுவுமில்லை

நோய்த் தடுப்பு மண்டலம் சரியாகச் செயல்படுவதை கால்-கை வலிப்பு மருத்து தடுக்கிறதா?

வலிப்பு சிகிச்சைகளுக்குப் பயன்படும் வலிப்பு எதிர்ப்பு மருந்துகள் என அழைக்கப்படும் தரமான மருந்துகள் நோய்த்தடுப்பு மண்டலத்தைத் தடைப்படுத்துவதில்லை.

என் குழந்தைக்கு கால்-கை வலிப்பு இருக்கிறது. அதிக ஆபத்துள்ளதா?

உங்கள் குழந்தைக்கு வேறு ஏதாவது சுகாதாரப் பிரச்சினை இல்லாமல் வலிப்பு மட்டுமே இருந்தால் கரோனா வைரஸால் அதிக ஆபத்து ஏற்படுவதற்கான சாத்தியம் இல்லை.

COVID-19 Vaccination in India

As of now, 19.03.2021, more than 1.6% (122,649,897 million) of the world population have been infected with COVID-19 with more than 2.7 million deaths.

Vaccination remains a main tool to help to prevent the further COVID-19 related illness and death and to control the Pandemic.

India, the second most populous country in the world, has administered more than 39.3 (39,339,817) million vaccines till 18th March 2021.

India began its COVID-19 vaccination programme on January 16, 2021, covering healthcare workers in the first phase. In early February, the vaccination programme was expanded to include frontline workers. In the ongoing second phase, which began on March 1, 2021, people aged above 60 years and those above 45 years of age with co-morbidities are being vaccinated.

Oxford University-AstraZeneca's Covishield vaccine, manufactured by Serum Institute of India, and Bharat Biotech's Covaxin are being used for the Indian vaccination programme.

India backs AstraZeneca COVID-19 vaccine, will continue to use it in COVID-19 immunization program.

As of 17th March 2021 more than 27 million doses of Covishield vaccine (AstraZeneca vaccine by Serum institute of India) have been administered in India

The AstraZeneca COVID-19 vaccine (including Covishield) continues to have a positive benefit-risk profile, with tremendous potential to prevent COVID-19 infections and reduce the COVID-19 related deaths across the world including in India.

As of 18.03.2021, in the global list of total number of COVID-19 cases, India is in third place after the USA and Brazil with 11,513,945 cases, and in the global total COVID-19 related death list, India is in fourth place after the USA, Brazil, Mexico with 159,405 deaths.

What Is Long COVID (PASC)?


Some people with COVID-19 continue to have lingering symptoms for weeks or months after they begin to recover. You might know this as “long COVID.” Experts have coined a new term for it: post-acute sequelae SARS-CoV-2 infection (PASC).

Research says about 10% of people with COVID-19 get long COVID. But it can happen to anyone whether you’re young, old, healthy, or have chronic illness. You can get it even if your early COVID-19 symptoms were mild to moderate, or regardless of whether or not you went to the hospital for them.

Causes

Experts don’t know why people get long COVID. 









Symptoms


Long COVID appears to affect each person differently. You might feel a wide range of things that could linger for a long time. 

Common symptoms include:Fatigue,brain fog,
loss of smell and taste,
shortness of breath,cough,
joint pain and chest pain.

You might also have sleep issues,anxiety,gut problems,
fever muscle pain,depression,
headache and rapid heartbeat.

In rare cases, long COVID can affect your organs and you may get inflammation of your heart muscle,lung-related issues,kidney problems,hair loss,skin rashes , sleep issues,
 concentration and memory problems,depression, anxiety, and  mood changes.

Prevention

The best way to avoid long COVID is to prevent COVID-19 infection.

You should wear a mask and maintain at least 6 feet of distance between yourself and other people when you're in public. Wash your hands with soap or use alcohol-based sanitizer as often as you can and avoid going to crowded places.

When a COVID-19 vaccine becomes available to you, you should take it. If you have questions or concerns about the vaccine, talk with your doctor about it.

COVID-19 Vaccination Drive Through Private Hospitals


On March 1st, 2021, our country has
launched its second phase of the Covid-19 vaccination drive for the people above 60 years of age and those above 45 years of age with comorbidities with a biggest vaccination campaign against COVID-19 infection. That day began with Prime Minister Narendra Modi being administered his first dose of the vaccine at AIIMS.

In Tamilnadu about 836 private hospitals
which have been empanelled under Chief Minister’s Comprehensive Health Insurance Scheme (CMCHS) or Pradhan Mantri Jan Arogya Yojana (PMJAY), have been signed up for administering the COVID-19 vaccination for the public from 1st March 2021.

In central districts there are about 114
private hospitals are available to administer vaccination against COVID-19 which includes over 50 hospitals in Trichy district, 23 hospitals in Thanjavur district, 13 hospitals in Karur district , 10 hospitals in Pudukottai district, 7 hospitals in Cuddalore district, 7 Hospitals in Perambalur district, 2 hospitals in Mayiladudurai district, and one hospital each in Nagapattinam and Ariyalur district.
Public in above age group can approach the private hospitals for COVID-19 vaccination with an affordable cost with a simple procedures of showing the Aaadhar card.

World is fighting with Vaccines to End the COVID-19 War.

At the end of one year after the World Health Organization (WHO) on March 11, 2020, declared the novel coronavirus (COVID-19) outbreak a global pandemic, as of 12.3.2021, in 219 countries, over 1.51% (119,228,648) of the world population are infected with COVID-19 along with 2,643,951 deaths.

European countries have recorded 26.35% of the total global COVID-19 cases with 28.68 % of COVID-19 related deaths.

Among the European countries, the UK leads the list in the total numbers of COVID-19 cases (4,241,677) and COVID-19 related deaths (125,168).

A higher number of daily cases were reported in France and of daily COVID-19 related deaths were reported in Poland.

As of March 9, 2021, the United Kingdom had the highest COVID-19 vaccination rate in Europe, having administered 35.02 doses per 100 people in the country.

The UK was the first country in Europe to approve the Pfizer/BioNTech vaccine for widespread use and began inoculations on December 8, 2020.

In the USA, COVID-19 cases have started declining and it has registered a single day spike of over 62k (62,773) cases and the total cases has now crossed the 29.9 million mark (29,925,902) with over 5.4 lakh (543,721) deaths (with the single day deaths of 1,531) on 11.3.21.

In India, about over 25 million (25,685,011) were vaccinated against COVID-19 as of 10.3.2021

Cases and death rates in India, Tamilnadu and in Trichy, my home town, are declining and our country's single day spike of COVID-19 cases was over 21 k (21,668) with the single day deaths of 113.

In India the total number crossed the 11.3 million mark (11,305,979) on 11.3.21, one year after WHO declared the COVID-19 Outbreak a Pandemic.

In India, recovered cases were second to the U.S and are over the 10.9 million mark (10,947,252) on 11.3.2021.

To bring this COVID-19 pandemic to an end, a large share of the world needs to be immune to the virus. The safest way to achieve this is with a vaccine. Now the world has vaccines to fight against COVID-19. Vaccines are a technology that humanity has often relied on in the past to bring down the death toll of infectious diseases.

The challenge now is to make these vaccines available to people around the world. It will be key that people in all countries — not just in rich countries — receive the required protection against COVID-19

Prof Dr.Aleem M.D.,D.M., (Neuro) a rural Neurologist / Neuroscientist was the Former Dean I/C vice principal HOD& professor of Neurology KAPV Government Medical college& MGM Government hospital Trichy. He is graduated from Madras University in 1980. He has received his MD., General medicine from Bharadhidasan University in 1986 and his DM,Neurology through the Tamilnadu Dr.M.G.M Medical University in 1999.ln 2001 as IAN sponsored candidate he has undergone training at University college of London' Institute of Neurology queen' s Square London in the field of epilepsy . He was the visiting professor for Annamalai University's Raja Muthaiya Chettiar Medical college and hospital chidambram. A visiting consultant Neurologist at Ordinance factory Govt of India Trichy . for 16 years from 2004 He is deeply interested in medical education and services since 1986. He has orated, presented and published more than 107 research papers in neurology and General medicine at various National and International, conference and Journals.

Now he is the Emeritus Professor at the Tamilnadu Dr.MGR Medical University of Government of Tamilnadu

He is also the senior consultant neurologist and epileptologist at ABC Hospital Trichy He is a visiting professor for Dhanalakhmi Srinivasan medical college and hospital perambalur since 2016.He is the visiting neurology Consultant for Apollo SpecialityHospital Trichy since 2016.

Course at Thirunelveli Govt medical college in 2007. He has published11 books in regional language on Neurology topics to create awareness among people. He is the examiner in the Tamilnadu Dr. MGR medical University. At present he is the president for Trichy Neuro club and Secretary for Neurologist association of Tiruchirappalli. COVID-19 Vaccines and COVID-19 Variants

In recent days the effectiveness of COVID-19 vaccinations is being impacted by the emergence of SARS-CoV-2 variants.

These variants could affect not only the effectiveness of the vaccines, but also the natural immunity that COVID-19 survivors have developed, making annual booster shots required for COVID-19 immunity.

Many natural COVID-19 variants have been identified, including the B.1.1.7 variant in the UK, the B.1.351 variant in South Africa, and the P.1 variant in Brazil.

Currently there is no indication that the B.1.1.7 UK variant reduces the effectiveness of Moderna or Pfizer COVID-19 vaccines.

Novavax reported that its vaccine had 85% efficacy with the variant versus 89% with the non-variant strain, and AstraZeneca/Oxford reported that their vaccine has 74% efficacy with the variant versus 84% with the non-variant strain.

The south African B.1.351 variant is potentially more troubling. Clinical trials for the Pfizer and Moderna vaccines indicate a reduction in sera neutralization of the B.1.351 variant, and other studies have shown there could be a six-fold to 10-fold lower binding affinity for antibodies to the variant.

Johnson & Johnson’s vaccine had 57% efficacy with the South African variant, where the variant is prevalent, versus 72% in the US, and Novavax’s vaccine had 49% efficacy in South Africa versus 90% in the UK.

Pfizer and Moderna are already working on developing booster shots for their vaccines to improve their effectiveness against the B.1.351 strain. As there is already some indication of COVID-19 reinfections and possible waning immunity from naturally recovered COVID-19 patients, yearly boosters may be required for COVID-19. However, it is important to note that despite the presence of these variants in the population, the vaccines are still likely to prevent severe outcomes such as COVID-19 related hospitalization and death.

Vaccines can offer a more consistent level of protection and a more robust immune response of neutralizing antibodies. Single-dose boosters provided to patients who have naturally recovered from COVID-19 could increase and extend the duration of their immune response.

But currently, vaccines developed so far may become less effective as new variants of the novel coronavirus spread widely, according to a new study.
