Movement disorders are some of the most complex diseases to treat, diagnose and understand. In order to increase global awareness of this sub-specialty among healthcare professionals and the general population, the International Parkinson and Movement Disorder Society (MDS) is launching the first World Movement Disorders Day on November 29, 2022.

Movement disorders are complex neurological disorders   with many misconceptions about them. Because of many misconceptions , people with these disorders get delayed medical treatment.

By increasing awareness of these difficult-to-diagnose diseases, it will be possible for more people to connect with professionals who have the specialized knowledge to provide proper diagnosis and management in time.
 

Among various neurological disorders, the subspeciality of Movement Disorders has been rapidly growing over the years both in relation to understanding of various diseases and therapeutic advances.

Movement disorder (MD)  implies abnormal movement or paucity of movement either voluntary or automatic which is not attributable to weakness or spasticity or any other medical causes directly interfering musculoskeletal system, such as, advanced rheumatoid arthritis or slowing of medical condition like hypothyroidism. 

It has been broadly subdivided into two types: Hypokinetic and hyperkinetic Movement Disorders.
 
The hypokinetic disorders are Parkinson's disease (PD) and Parkinsonism plus syndromes.


The hyperkinetic types are dystonia, chorea, ballism, athetosis, tics, tremor, myoclonus and stereotypes. 




Movement Disorders constitutes 3-8% of neurological disorders in India with a crude prevalence rate (CPR) varying from 31 to 45/100,000 above 60 years of age and they are more frequent in urban and rural India. 


Hospital based study has shown that MDs account for 20% of all neurological diseases. 

At Trichy,   in my patients with neurological disorders around 15% of the patients are with movement disorders. 

In the children chorea and tics are more common in our region

Among elderly Parkinson diseases are more frequent 
In our Trichy region.

In adolescents and middle aged group, functional movement disorders and tremors are more come in our area.

In the recent smart phone era, task specific dystonia dyskinesia are frequently noticed in adolescents and in adults.


Movement disorders can result from many types of brain injury, such as head trauma, infection, inflammation, metabolic disturbances, toxins, or unintended side effects of medications. They can also be a symptom of other, underlying diseases or conditions, including genetic disorders and aging .


In the treatment of movement disorders, considerable therapeutic advances have been achieved seeking a more efficacious and durable therapeutic effect. 


Treatment of movement disorders varies from person to person, depending on the type of disorder and the underlying cause. Some  with movement disorders may only need observation. In children , many movement disorders improve as they develop.


When treatment is necessary, options it may include:

Oral medications to relax muscles and ease stiffness, tremors, and spasms

Injections of botulinum toxin (Botox) for dystonia that affects a single body part, and sometimes for tic disorders and myoclonus

Dopamine (levodopa) for parkinsonism

An implanted baclofen pump can be helpful for dystonia mixed with spasticity that does not get better with oral medications

Deep brain stimulation is a treatment for primary dystonia that does not respond to medications

Patients with movement disorder may also benefit from:

Physical therapy to strengthen or stretch the affected limbs and muscles

Occupational therapy to help overcome any limitations caused by abnormal movements

Support to help the family and child cope with ongoing symptoms of movement disorders

Neuropsychiatric testing, when the movement disorder is accompanied by cognitive or behavioral problems



Movement disorders constitute a major burden among the neurological disorders. Awareness about these disorders is important to get early treatment and it's benefit.

Trichy city is in the centre part of Tamilnadu with better connectivity by road, rail and air. In the Recent months the city infrastructure development is overwhelming .
To improve the traffic, Trichy is going to have more flyovers and bridges.  

From Srirangam Island to trichy main land we are going to have a new high level bridge along with the existing one  across the river Cauvery.

The second flyover is between Melachinthamani to Mallachipuram and the third flyover is from head post office to MGR statue .

The fourth flyover is the longest one  from Melachinthamani to Tiruchirappalli railway junction.

The fifth bridge is going to be reconstructed is the railway bridge near Maris theatre complex .

The sixth one is nearing completion at Mannarpuram end of  junction road over bridge .

The seventh one is the new road over bridge after demolishing the existing very old railway bridge to interlink railway junction,  central bus stand ,Crawford,Trichy- Dindigul and Trichy -chennai highways.

This  Road over bridge will have four approach lines from Trichy- chennai National highway, Kallukuzhi, Trichy railway junction and Aristo circle .

Along with these one more road over bridge is required  on the National highway near Sanjeevi nagar  for easy entry of vehicles in to Trichy city via Oyamari crematorium road.

With the ongoing  integrated bus terminal projects, these new bridges and flyovers will definitely help to reduce traffic congestion in the Trichy city.

Rain our region brings us life and energy. The monsoon season brings that much-needed relief to the scorched land. However, besides being very refreshing, the heavy rains also provide a breeding ground for mosquitoes and a humid environment, which in turn can lead to rapid spread of microbe-borne diseases. Diarrhoea, Malaria, dengue, flu, chikungunya, leptospirosis, hepatitis etc. are some of the common health conditions that pose a threat to our health during the rainy season.

Rainy season precautions that can be followed during the monsoon to stay healthy and keep infections at bay while enjoying the rains are as follows:



*Drink clean and boiled *water
Avoid street food and junk food
*Avoid stagnant water around the house
*Wash fruits and vegetables carefully
*Follow an exercise regimen
*Wash your hands
*Avoid getting wet in the rain too often

Brain attack (stroke ) is the second most common cause of death and a leading cause of disability. Stroke has a yearly global incidence of over 15 million patients and is responsible for over 116 million disability-adjusted life years (DALYs), of which an estimated 16·9% can be attributed to air pollution. 

Increasing evidence suggests that air pollution is an emerging risk factor for stroke. Over the past decades, air pollution levels have continuously increased and are now estimated to be responsible for 14% of all stroke-associated deaths.

There is a clear association between air pollution and the different causes of ischaemic stroke and haemorrhagic stroke. The risk for ischaemic stroke is increased after short-term or long-term exposure to air pollution. This effect is most pronounced in people with cardiovascular burden and stroke due to large artery disease or small vessel disease. Short-term exposure to air pollution increases the risk of of  intracerebral haemorrhage, a subtype of haemorrhagic stroke.



The incidence of air pollution-related stroke is most pronounced in low-income and middle-income countries due to their fast-growing economies, urbanisation, and industrialisation.

In Our city, monitoring of the
air quality has revealed that the air quality index (AQI) has worsened from satisfactory to moderate level due to air pollution. 

Our corporation  is doing  many mitigation work to prevent air pollution in the city by creating more parks , by planting trees and by Increasing the city green spaces. 

People should also cooperate to prevent air pollution in the city. They should not burn the garbage in open places.  City People   should do home garden terse garden and indoor plantations to mitigate air pollution .

 In this world stroke day 2022 October 29th People should in some way or other help to prevent air pollution in the city for a healthy Brain
