Early Winter and Election are causing a Second Wave COVID-19 Spread in Europe and the USA
In the 44th week of the COVID-19 Pandemic, about 216 countries and territories around the world and 2 international conveyances have so far reported COVID-19 cases.
The global confirmed COVID-19 has crossed 45 million.
During election 2020 and in early winter and as a possible second wave, more and more COVID-19 cases are reported in the US and in Europe.
The world has registered a highest single day spike of over 5.45 lakh (545,936) COVID-19 cases on 29.10 2020.
Every second, one new COVID-19 case has been added to the total number of COVID-19 cases in the USA.
The USA has registered a single day spike of the highest ever number of 91,530 COVID-19 cases and the total cases have crossed 9.2 million mark (9,212,767) with 234,177 deaths as of 29.10.20.
In Europe over 250,000 cases were registered in a single day on 29.10.2020 and the maximum number of cases (47,637) is reported in France.
India is in second place after the USA in the list of global single day spike of COVID-19 cases with over 49k (49,281) on 29.10.2020 during this Pandemic.

Trichy: Locals and experts have appealed to the authorities to preserve the British-era steel bridge across the Cauvery river that connects Trichy mainland with Srirangam island, instead of the proposal to dismantle it. Residents said the bridge can be strengthened and utilised as an exclusive public space for walkers, as the city has very limited public space for leisure.

Recently, the state highways department proposed to dismantle the bridge based on an expert study. Reacting to this, locals and experts said that the 91-year-old bridge must be preserved as a heritage structure alongside the old Kollidam bridge.
“There is a scope for rehabilitation. Modern heritage is as much as important as ancient heritage. The bridge should not be dismantled,” S Vijayakumar, architect and member of Indian Institute of Architects (Trichy centre) said.
The institute is willing to offer technical support for the rehabilitation drive as the piers, if strengthened, can be utilised for walking purpose, he added.
“Information boards mentioning the technical details of the bridge will help students,” Dr M A Aleem, member, Trichy district welfare fund committee said.
The rehabilitation of the bridge as a public space will also send a message to conserve the heritage, residents said.

The COVID-19 situation in the US may affect the US Presidential Election 2020.
In the 43rd week of the COVID-19 Pandemic, as of 24.10.2020, the total number of global confirmed COVID-19 cases has crossed the 42 million mark (42,469,950) and the total COVID-19 related deaths are over 1 million (1,149,142). Over 31 million (31,423,036) have recovered from this illness.
In India, as of 23.10.2020, there are about 7,813,668 confirmed COVID-19 with 117,992 deaths. The death rate in India is as low as 1.51% (which is less than that in the USA). The recovery rate is 89.51% (which is higher than that in the USA).
In the USA the number of COVID-19 cases are rising. The US registered a high single day spike of COVID-19 cases of 81,210 cases on 23.10.2020
On 23.10.2020, the USA has registered 8,746,953 total COVID-19 cases with 229,284 total deaths. The recovery rate in the USA is 65.29% and the death rate is 2.63%
In Europe COVID-19 cases are rising and the single day spike of COVID-19 cases were over 2.3 lakh (231,396) on 23.10.2020
In the 1st and final Presidential debate for USA election 2020, more time has been taken to discuss the COVID-19 situation in the US, and this may be an important factor in decision making in this 2020 US presidential election.

While Kendriya Vidayalayas and Jawahar Navodaya Vidyalayas are preparing to have regular classes from November 2 by following the Standard Operating Procedure, there are indications that many private CBSE schools in Tiruchi are not keen on conducting physical classes.
In deference to the directions of the Central Government, the Kendriya Vidyalayas are in the process of collecting feedback from parents on conducting regular classes for levels IX to XII.
“Of course, we are ready to conduct classes. But then, the parents have the say on this matter. By and large, parents seem to be reconciled to the outcome of the online classes, and are not prepared to take the risk of sending their wards to the schools,” the principal of a Kendriya Vidyalaya said.

The Kendriya Vidyalayas had completed half-yearly exams earlier this month, and the short week-long vacation has also begun. Multiple choice questions were asked for the online exam of one-hour duration for each subject, he said.
As for the private CBSE schools, the standing instructions of the State Government also have to be followed.
The private school managements are wary of starting regular classes after what they perceive as the “risky experimentation” by Andhra Pradesh Government to open schools. The move had turned counter-productive, leading to further spread of the pandemic, school principals say.
Since the CBSE schools had started the online classes early on, they have been able to complete close to 80% of the portions, after factoring in the 25 % cut in contents this year due to the pandemic.
“Nevertheless, we cover the portions deleted for this academic year also, keeping in mind the need to keep the students fully prepared for competitive exams for entry into professional courses,” said C.J. Chacko, principal, Samad Senior Secondary School.
Following the Central Government’s plan for re-opening of schools the Anna Science Centre - Planetarium in Tiruchi had held an awareness programme for teachers and parents on Tuesday.
The programme titled ‘Meet your Medical Expert’ programme was conducted to allay apprehensions in the minds of the parents, R. Ahilan, Project Director of the Anna Science Centre, said.
Interacting online with parents and school teachers, M.A. Aleem, former Vice-Principal, K.A.P.Viswanatham Government Medical College Hospital, Tiruchi, said students must desist from sharing pencils, pens or erasers, as an extreme precaution to prevent the spread of the pandemic.
Vaccine is the only hope, and till it comes the practice of wearing masks, washing hands frequently and maintaining physical distance must be assiduously followed, Dr. Aleem said.

COVID-19: A Global Health Calamity
In 2020, the COVID-19 pandemic is considered as the most dangerous global health calamity of the century and the greatest challenge that humankind has faced since the Second World War.
As far as the history of human civilization is concerned, there are instances of several severe outbreaks of diseases caused by a number of viruses.
As of now on 17.10.2020 in the world about 39,633,011 are infected with the COVID-19 infection with about 1,109,833 COVID-19 related deaths.
The single day COVID-19 cases in the world have reached a new peak of over 4 lakh (413,206) with 6190 deaths on 16.10.2020
The USA has registered 71,687 cases in a single day and leads the global single day cases list with about 928 deaths on 16.10.2020
The USA tops the global COVID-19 cases list with 8,288,278 confirmed COVID-19 cases and 222,644 deaths on 16.10.2020
India is in second place after many weeks in the list of global single day spike of COVID-19 cases with over 65k (65,126) and 886 deaths on 16.10.2020 during this Pandemic.
As of 16.10.20, India is in second place in the global COVID-19 cases list with 7,430,635 cases.
At present coronavirus COVID-19 is affecting 215 countries (Wallis and Futuna is the 215th country added on 16.10.2020) and territories around the world and 2 international conveyances in almost in all continents.
Till now there is no report of any clinically approved antiviral drug or vaccine that is effective against COVID-19. It has rapidly spread around the world, posing enormous health, environmental, political, racial, social, religious and regional challenges to the entire human population. The coronavirus outbreak is also severely disrupting the global economy.
Almost every nation is struggling to slow down the transmission of the disease by testing & treating patients, quarantining suspected persons through contact tracing, restricting large gatherings, maintaining complete or partial lock down, etc.
The surge of new COVID-19 cases in the 42nd week of the Pandemic in the European and American continents and in the Indian subcontinent requires more attention.

Covid treatment simplified: For doctor's

*Home treatment* :
* Tab Vit C 500 three times a day
* Tab Vit B with zinc twice a day
* Tab Vit D 60000 once in a week.
* Can add ayurvedic/ homeopathic medicine which may improve Immunity.
* Tab Ivermectin 12mg twice a day for 5 days. ( not yet recommended by ICMR)
* Azithromycin/ antibiotics / Paracetamol etc as per need.
* Those at high risk / elders
Tab Favipiravir (200)
8-0-8 day 1
4-0-4 day 2 to 5.
( keep watch on Oxygen saturation : admit if high risk/ elders or saturation below 95%)
*Hospital treatment* : With low saturation
* Steroids :
Dexa 4mg tds/ MPS 40mg tds for 5-7 days.
Oral steroids: if required thereafter.
* Anticoagulants :
LMWH along with steroids for 5 to 10 days
Oral anticoagulants if long term oxygen is required beyond 10 days.
( check D-dimer)
* Remdesivir : to start as early as possible. ( happy hypoxic)
Preferably all patients requiring oxygen and at high risk. ( O2 level / age/ Co morbidity / CT score)
Role out contraindications.
* Tocilizumab/ itolizumab. ( breathless patients with increased respiratory rate: no longer happy with low saturation/ Cytokine storm) rule out any secondary infections.
* Plasma ( Indication similar as for tocilizumab: if possibility of worsening ) start as early as possible.
* Supportive treatment :
Oxygen / bipap / ventilator /Antibiotic / fluids / inotropes / dialysis etc.
* Tab ivermectin 12mg
twice a day for 5 days, may continue once a day for next 5 days.
Higher dose of 2-0-2 in hypoxic patients to be tapered off after improvement
( not recommenced by ICMR)
* Additional medicines used :
Antiplatelets / Statins / Monteleukast / acalabrutinib and similar / nizonide / and others.
*Most drugs are experimental drugs which needs to be used keeping in mind their risk : benefit ratio*.
Please follow ICMR guidelines which is based on scientific inputs.

Follow precautions scrupulously
While most people afflicted with COVID-19 have recovered quickly, for some, the symptoms linger for weeks or months following the initial recovery.
These patients develop complications that may have lasting health effects, damaging lungs, heart and brain.
Older people and people with many serious medical conditions are the most likely to experience lingering COVID-19 symptoms.
The potentially long-lasting problems from COVID-19 make it even more important for people to reduce the spread of the disease by following precautions such as wearing masks, avoiding crowds and keeping hands clean.

In the 41st week of the COVID-19 Pandemic, the world registered the highest single day spike of 348,681 cases on 8.10.2020. The world has registered about 4,713 total COVID-19 cases per 1 Million population and 136.8 deaths per 1 million population as of 8.10.2020.
In Europe, a spike of about 90,859 COVID-19 cases were reported in a single day with the highest number of cases (18,129) from France on 8.10.2020.
In North America, on a single day about 68,256 cases were reported with the maximum cases of 56,652 from USA on 8.10.2020.
In Asia, on 8.10.2020 about 108,848 cases were reported in a single day with the maximum number (70,824) from India.
In South America, about 59,015 new cases were recorded in a single day on 8.10.2020 with a maximum number (27,182) from Brazil.
In Africa, new cases of 10,770 has been reported in a single day on 8.10.20 with the highest number of cases (1,736) from South Africa.
In Oceania, only 97 new COVID-19 cases were reported without any newer death in a single day on 8.10.2020.
Without vaccines, the Oceanic region has the lowest number (less than 100) of new COVID-19 cases with nearly zero deaths in the 41st week of the COVID-19 Pandemic.

US President Donald J. Trump Tested Positive for COVID-19
Social Distancing, Personal Hygiene and wearing of Mask will Keep You Away From COVID-19 with or without vaccines
In the 40th week of the COVID-19 Pandemic, as of now on 2.10.2020, around 0.44 % of the world population is infected with COVID-19.
In USA on 1.10.2020 , about 7.4 million (7,494,671) people are confirmed with COVID-19 infection along with over 2.1 lakh (212,660) deaths .
On Thursday (1.10.2020) COVID-19 is confirmed in US President and in his wife.
My thoughts and prayers for a speedy recovery of US President Donald J.Trump and his wife from a asymptomatic stage of COVID-19 infection.
I am scared to hear that the world most important person's (USA President Trump 74 years old) security of many layers are penetrated by the
Coronavirus on 1.10.2020. He himself tweeted that he and his wife were infected with COVID-19. I don't know whether it due to President 's security lapses in preventing the biological attack during this COVID-19 Pandemic or it is due to a inadequate personal protection and hygiene.
After a spike of 8513 COVID-19 deaths on 17th of April ,Only on yesterday ( 1.10.2020) world has recorded the ever highest single day death of 8934 cases with higher number of deaths from Argentina which is about 3352 .
India is in the top for the past many weeks in the list of global single day spike of COVID-19 cases which is 81,693 on 1.10 2020 with 1,096 deaths .
As of now on 2.10. 20 in 213 countries and territories in all continents and in 2 International conveyances in the world has recorded a total number of over 34.6 Million cases (34,622,646) .
In India over 6.4 Million (6,438,968) total COVID-19 cases are recorded which include 608,885 cases in the State of Tamilnadu and 10,708 cases in my home district of Trichy as of on 1.10 2020.
The global recovered cases are 25.7 Million (25,773,764) and COVID-19 related deaths are over 1 million ( 1,029,764) which include over 1 lakh deaths (100,323) in India and 9653 in Tamilnadu and 151 at Trichy as of now on 2.10.2020.
